Your IEP team is required to create a transitions plan no later than the student turning 16. A coordinated set of activities will be planned to help the student reach their self-identified post-secondary goals in the areas of employment, education/training, and independent living (if appropriate).

About the event
Your IEP team is required to create a transitions plan no later than the student turning 16. A coordinated set of activities will be planned to help the student reach their self-identified post-secondary goals in the areas of employment, education/training, and independent living (if appropriate). Students will take different transition assessments, including interest inventories, employability skills surveys, student interviews, and workplace readiness assessments. Students will identify job interests and be aware of their abilities and skills that will help them to be successful. Activities include community-based instruction, career exploration, job sampling, daily living skills, and employability skills instruction. A graduation plan is usually created with the transitions plan and it is important for the student and parent to drive this process so the student is taking classes they need to be successful in their identified path.
